Infertility is defined as the inability to get pregnant or carry a pregnancy to full term after trying for 12 months if under 35, or 6 months if over 35. It affects an estimated 1 in 8 couples in the US.
There are many potential causes of infertility:
- Ovulation disorders, meaning the woman's body does not release an egg regularly. This accounts for 25% of female infertility cases.
- Blocked fallopian tubes, which prevent the egg and sperm from meeting. This may be caused by pelvic inflammatory disease, prior surgeries, or endometriosis.
- Endometriosis, where uterine tissue grows outside the uterus, affecting up to 50% of infertile women.
- Uterine or cervical issues like fibroids or an abnormally shaped uterus.
- Primary ovarian insufficiency, when ovaries stop working normally before age 40.
- On the male side, low sperm count or motility, which makes up 30-40% of infertility issues.
Diagnosing infertility starts with an evaluation of both partners’ full medical history, often followed by hormone testing, ultrasounds, x-rays to view the uterus/fallopian tubes, and semen analysis for the male.
Treatment depends on the cause:
- Medications to stimulate ovulation or regulate hormones
- Surgery to repair reproductive organs, remove scar tissue/fibroids/endometriosis tissue
- Intrauterine insemination (IUI), placing washed sperm directly into the uterus around ovulation
- In vitro fertilization (IVF), involving fertilization in a lab and transfer to the uterus
